Appointment Request Form

If you are a first time client, please start by texting or calling 512-575-3231.

This is the quickest way to begin our intake process and get on the schedule.

​

If this is an emergency, please call 512-575-3231 and press 1 to connect to our emergency line, leave a voicemail if no one answers.

Our voicemails are monitored for emergencies.

If you are a returning client you may use the  Appointment Request Form. Calling, texting or emailing can also be used to make an appointment, however forms are given priority.

If you have multiple pets for one visit, please additionally fill out the Multiple Pets Information Form afterwards.

Medication Refill Request Form

Already a client of Matthews Vet Services? Submit this form to request a refill of your pet's current medication(s). Once we receive the request and the refill is approved, we will email you an invoice. The medications will be available once the invoice is paid.
